• Submit form via fax to 573-882-1953 with hard copy of approved purchase order/billing authorization or credit card information
• Mail form with check (payable to University of Missouri), hard copy of approved purchase order/billing authorization or credit card information to:

*By registering you give permission to distribute your name and contact information to conference attendees and vendors.  If you prefer not to be included in these distributed lists, submit a written statement requesting that your name and contact information to be omitted.  Please fax to 573-882-1953 or email muconf1@missouri.edu NO LATER than July 3, 2009.*

Refund Policy:  All cancellations and requests for refunds must be submitted in writing to the MU Conference Office at the fax, email or address above.  Requests received on or before close of business on July 2^nd will be granted a full refund.  If the request is received between July 3^rd-8^th a $25 processing fee will apply.  No requests for refunds will be honored after July 8^th; however, we do welcome substitutions.
